Nursing Care
We understand that staying in hospital can lead to anxiety for patients and their families. We also know that most hospital patients would rather prefer quick discharges from hospital to continue their recovery in the comfort of their own homes. UPL offers nursing services to clients that are aging, have disabilities or have chronic care needs so they may maintain their freedom in personal or health-related concerns.
Qualified and caring nursing staff oversee and direct all of our medical care services, using customized Care Plans for most clients. Our dedicated nursing professionals includes RNs, LPNs and NARs. All are dedicated to the high standards of professional and compassionate care that have allowed UPL to continue to achieve and maintain exemplary status in Minnesota.

Case Management
The purpose of case management services is to help patients handle aspects of their lives that are not necessarily related to substance abuse but that might impact whether the patient remains in treatment or has successful treatment outcomes. These services provide assessment, planning, coordination, monitoring, and evaluation of options and resources to meet an individual's specific needs. Some examples of case management services might be:
Assisted Living
Assisted Living is defined as, "housing for the elderly or disabled that provides nursing, meals, and/or housekeeping as needed."
For those who are able to care for themselves independent living facilities are an option that can provide help with activities of daily living (ADLs) like laundry, bathing, and help with errands.
For temporary care in the home setting at-home nursing care can be an option, and for those suffering with dementia or Alzheimer's special memory care facilities are available with specially trained staff. In-Home Care
Providing care for a loved one that keeps them safe and maintains their quality of life in the home setting is referred to as in-home care. Care can be provided by a family member or trained personnel. Many patients prefer receiving care in the home setting because it is much more private and comfortable. The type of care given will vary widely from person to person. Some patients will only need someone that comes to the home a couple of times a week to do some light cleaning and possibly assist with running some errands. Care that is provided for non-medical purposes is referred to as in-home care.
Sometimes licensed personnel are needed to provide home health services which are more skilled in nature such as providing assistance with weekly medication set up. Some patients may even require complete 24-hour home health care for various medical needs. Both in-home care and home health care are becoming increasingly more popular options for providing a cost-saving method of receiving the care you need.
Medication Management
Elders take more medications than other age groups and benefit the most from them. Regrettably, older adults also have the highest risk for not taking medication accurately. The consequences of medication mistakes can range from mild to severe adversarial reactions and sometimes even death. UPL is here to help provide peacetime for you and your loved one by administering medication reminders and assisting with medication management.
Respite Care
Respite care gives you time off when you need it. Our respite care services provide care for all persons irrespective of their age, culture, language, disability and/or medical condition.
Respite care is available from a few hours at a time to a few weeks at a time, and can be provided in your home, in one of our supported accommodation sites, or somewhere else you think is appropriate.
We make life comfortable for those under our care and provide a range of stimulating activities to keep them engaged.
